[epub] Advertising Rates and Contract?

Dear List,

I have been reading your discussions with great interest.  I produce and
publish two email newsletters -- one for crafters
http://www.heartlandtreasures.com/currentissue.htm; and one newsletter for
K-12 teachers http://www.applefortheteacher.com/currentissue.htm.

Although my websites and newsletters are very new, I am already receiving
inquiries from individuals who would like to advertise in my newsletters.  I
have my advertising rates posted on my website but I am wondering about the
need for a "contractual agreement" between me and the people who will be
advertising in my newsletters.

Do you know of a site that could provide a good model for an advertising
agreement?  Is such a document necessary?
