From a personal perspective, I like HTML ezines
as long as I can read them online or off.  They are
much easier to read and far more robust and
eye appealing.  They seem to accommodate ads better.  
So many text ezines / newsletters are littered (and 
I MEAN littered as in major eye trash -- a subject
for another day I guess) with ads these days that 
it makes it hard to find the good stuff. 

Anyway, there's a chapter on HTML ezines in
Chris Pirrillo's book "Poor Richards E-mail
Publishing" that's pretty good.  He's been
publishing "Lockergnome" in HTML for a couple
of years and would probably be a good source

You might also try Shannon Kinnard's "Marketing
With E-Mail."  She has a small section on HTML
newsletters in her book.  She publishes the weekly
"Marketing With Email Idea Feed" and may also
be willing to give her thoughts on your questions.
