Has anyone used the Make Your Price Sell survey(s) for ebooks?

I've seen it recommended several times but - maybe I'm just dense or more
mistake prone than most - I can't seem to figure out how to get a useable
survey out of it.

When I first tried it we didn't have a name or email set up and the Thank
you email ended up with my personal email address as sender and 'Not
named yet' as the 'signature'.  I've used both the feedback and the email
address given to ask how to correct those mistakes and never got an
answer.

If anyone has used it - can you use categories such as poetry, short
story collections, novels, craft of writing, chat guides and get some
guide to pricing from the survey?

The 'free' download has some really interesting concepts and I'd sure
like to try out the surveys.
