It's the simple things that trip us up sometimes.  Unless you
published an article or story without the author's name
credited, you're fine.  That's all a by-line is,
basically...just the author's name (or pseudonym if they use
one).  

Now, about the simple things...I forgot to add when I replied to
the volume query.  I worked for a large publishing/distribution
house and we did our volumes as ...year..A,  same year B. which
would've been annually.  Didn't mean to steer anyone wrong. Sooo
sorry!    Now if I could stop proving that I'm human, I could
reach for the stars???  :o)

An excellent reference text for new publishers or writers is
"Writing A-Z"  by Kirk Polking.  Not sure if it's in a current
edition published by Writer's Digest, or not.  Even an older
version would be helpful for new to the fielders.
