Debbie,

Although it would require you to change list hosting services (assuming
you're using one now), EmailXtreme offers this as one its features.  It
is completely automated and allows you to generate reports of the
recommendations from any time period of your choosing (i.e. last 30
days, this year, etc.)

If you were to try to implement this yourself or contract it out
however, I would recommend that you not use PIN's as used by the
solution recommended earlier. Instead simply use the recomendee's email
address. It is just as unique and allows easy matching of the
recommender to recomendee.  Also, in cases where the recommendation is
made verbally, the recomender is more likely to remember or have on file
the recommendee's email address than both of them remembering an
arbitrarily assigned PIN.
