Ok, here is what I did.

1. Go to something like http://openrbl.org/ or use the ping
command in MS-DOS.  Optionally run a trace route to your website.
Either ping yourdomain.com or tracert yourdomain.com.  The
openrbl.org site will however show you what network you are on.
For example, ezine-tips.com is on the Qwest network.  You could
then use arin.net to look-up your IP and see who the IP is listed
with.

2. Contact that network by going to their website and finding the
phone number.  When you get them on the line give them your IP
address and ask if you are hosted with them or if you should
contact someone else.  I'm on the nac.net network, but the
hosting provider is pwebtech.net, and the host is a reseller for
pwebtech.net.

3. Contact the company they refer you to and explain that you are
a permission based e-mailer.  Explain how you collect e-mail
addresses, etc.  They will probably ask you to send them and your
host an e-mail restating everything.  I would include a link to
your privacy policy, your business phone number, the legal
business name, and an e-mail address.

Ask that they contact you promptly if they ever receive any
Spam/UCE complaints on your company so that you may resolve the
issue immediately.

This is just what I did.  Some hosts may be harder, some may make
it easy, others might just blow you off.
